About Online Registration

Dr. Weinberg's office is one of the first ever to let you fill out your initial office paper work (Patient registration and Orthopedic History forms) directly online in a safe, secure, encrypted method. Online registration can save you up to 20 minutes on your visit time as well as allow you to type information instead of filling it out by hand.

If this is your first time using online registration, please review our statement of confidentiality regarding your medical information

Please note the following two important instructions:

1) Do not use your browser's back button to navigate between parts of the form. Use the tab buttons at the top instead or the next and back buttons at the bottom.

2) You must press"send and save" on the finish page to send your form! If you fill out the first page and close the browser, nothing will be sent to the office and you will have to start over.
